What to expect
01
INITIAL CONVERSATION OR REFERRAL
We discuss the participant’s goals, support needs and preferred exercise environment.
02
PRE-VISIT HOME RISK SCREENING
Before an initial home appointment, a home and environmental risk screening is completed with the participant, family member or support person.
This helps identify access requirements, pets, smoking, parking, mobility considerations and any other factors needed to plan a safe visit.
03
ASSESSMENT AND GOAL SETTING
The Exercise Physiology assessment explores current capacity, goals, preferences and suitable approaches to movement.
04
INDIVIDUALISED SESSIONS
Sessions are tailored to the participant and may occur at home, in a gym, at a pool or within the community.
05
PROGRESS REVIEW AND REPORTING
Progress is reviewed over time, with reporting and collaboration provided where appropriate.
A brief environmental check is completed again on arrival, as conditions within the home can change
Sometimes getting to an exercise session is a barrier in itself
Where appropriate, limited support assistance may be provided alongside an Exercise Physiology appointment to help a participant access their session safely and confidently.
Travelling with a participant to a gym or pool
Supporting completion of the prescribed exercise program
Assisting the participant to return home safely
This assistance supports access to Exercise Physiology and is not intended to replace a participant’s regular support-work arrangements.
