Ready to make a referral?

Referrals are welcome from participants, families, support coordinators and health professionals. If you already have the information needed to make a referral, you can complete the Kooka Support Services referral form below.

What happens next?

1. Submit referral
Complete the secure online referral form.

2. Initial contact
I’ll get in touch to discuss needs, goals and availability.

3. Home risk assessment
A home risk assessment is completed before any home-based visit.

4. Initial EP assessment
We meet in the most appropriate setting for an initial assessment.

5. Your program
An individualised program is developed around the person’s goals, abilities and preferences.

Information to have ready

  • Participant’s name and contact details

  • Preferred method of communication

  • NDIS number and plan dates, where applicable

  • Funding management type

  • Relevant goals and reasons for referral

  • Important health, mobility or support information

  • Details of the person’s support coordinator or relevant support team

  • Preferred appointment setting or location

Home-based appointments

Before attending a participant’s home for an assessment or appointment, Kooka Support Services will complete a home risk assessment. This helps identify any access, environmental or safety considerations before the visit.
